Total Care
If you are recovering from surgery, illness, wound, stroke or other disabling event, your doctor may prescribe home healthcare for you. TCNS consist primarily of Skilled Nursing, Certified Home Health Aides and Physical, Speech and Occupational Therapy services. These services are provided by a licensed home healthcare agency in the privacy and comfort of your own home and can only be prescribed by your primary care physician as part of a written plan of care.

As your home health provider, TCNS will coordinate and manage the treatment plan your doctor ordered for you. Our clinical staff will contact you throughout your treatment period to assign your nursing team, schedule nursing and therapy visits, or deliver medical supplies and equipment. During your care, we will monitor your progress and keep your doctor up-to-date on how you are doing.

You should contact us immediately if there are any changes in your physical condition or your environment. We encourage your participation in every aspect of the decision-making regarding your care.
